Jacksonville Memorial Hospital and Morgan County Health Department are urging residents to complete a survey about health concerns in their communities. This survey aims to help develop a plan for resources to address major areas of concern, as part of the Community Health Needs Assessment required by the Affordable Care Act. The results of the assessment will guide initiatives and efforts for the next three years.
The survey can be completed through the Memorial Health website, or a paper version can be obtained by contacting Claire Peak, Jacksonville Memorial Hospital community health coordinator, at 217-479-3928.
